Humana Expanding Value-Based Home Care Subsidiary to Virginia

Expansion will support Medicare beneficiaries healing at home and create employment opportunities in Virginia

June 21, 2022  – Humana recently announced that it will expand its value-based home care subsidiary, onehome, to Virginia. onehome will coordinate in-home care services for Humana Medicare Advantage beneficiaries in Virginia. The coordination will include working closely with existing home care providers to ensure that care is quality and convenient.

 The integrated model that onehome utilizes, creates a single point of accountability that accounts for the needs of patients, physicians, hospitals, and health plans for home care. Care will be delivered through a network of local home care providers. 

 onehome coordinates a variety of services at patients’ homes, including infusion care, nursing, occupational therapy, physical therapy, and durable medical equipment (DME) services. The goal is to make home care easier and more accessible for both members and physicians.

Humana has continually been expanding home care services over the last four years. Through the acquisition of Kindred at Home, now being rebranded as CenterWell Home Health, and organic growth, the company has grown to one of the largest providers of home care services. 

The expansion into Virginia is the next step in their growth trajectory.

 The onehome value-based model is rolling out in Richmond, Roanoke, and Southern Virginia counties to begin. After several months, it will be able to coordinate home care services for Medicare Advantage plan members throughout the entire state, as well as North Carolina.

This expansion is just the next step for Humana, with more to come in the future. Within 5 years, the company expects that 50% of all its Medicare Advantage plan members to have access to the value-based home care model.
