Primary Care First Model Application Deadline is May 21st

CMS announces the deadline to apply for Cohort 2 of the Primary Care First Model is May 21, 2021

May 19, 2021 – The Centers for Medicare & Medicaid Services (CMS) has recently sent out a reminder about the Primary Care First Model (PCF). The deadline to apply for Cohort 2 of the Primary Care First Model is May 21, 2021.

Interested parties must submit their application on the Primary Care First Online Application Portal by the deadline to be considered.

Primary Care First Model Details

The Primary Care First Model is a set of voluntary five-year payment options. It is structured to reward value and quality. The PCF Model does this by providing an innovative payment structure that supports the delivery of advanced primary care.

This multi-payer model is designed to allow for practitioner independence. It provides flexibility, allowing primary care providers the freedom to innovate their care delivery approach based on their resources and specific patient population.

There are five primary care functions within PCF:

  1. Access and continuity;
  2. Care management;
  3. Comprehensiveness and coordination;
  4. Patient and caregiver engagement; and
  5. Planned care and population health.

Primary Care First Cohort 1

There are 2 cohorts for PCF. The first Cohort began in January 2021. There are 827 practices and 14 payer partners participating in Cohort 1 of Primary Care First.

Currently, Primary Care First model is offered in 26 regions across the U.S., including the Great Philadelphia region of Pennsylvania, statewide in New Jersey, and in select areas throughout New York State.
