January 18, 2022 – Humana announced a partnership with Vancouver Clinic to offer value-based care services to Medicare Advantage enrollees in Gresham, Oregon.

Through the primary care practice, Enliven by Vancouver Clinic, residents will be able to receive care. Enliven will accept all Humana Medicare Advantage HMO and PPO plans offered in Oregon.

“We are grateful for the opportunity to bring Enliven to Gresham, which will improve access to quality care,” said Jesse Gamez, Intermountain Region Medicare President for Humana. “By combining Humana’s strong focus on improved member experience and clinical outcomes and Vancouver Clinic’s long-standing commitment to patient care, we are able to offer primary care that is dedicated to helping our members achieve their best health.”

Enliven by Vancouver Clinic Offers Unique Services

Enliven is part of an ever-growing value-based care partnership between Humana and Vancouver Clinic. The primary care practice serves older adults with complicated health concerns with the goal of providing whole-person care that supports a full health picture.

The company website touts its ability to provide exemplary care for older adults that goes above and beyond, including offering:

  • Longer appointment times with your doctor
  • Healthy living and exercise classes
  • Counseling
  • Community connection

Currently, there is only one Enliven location, in Gresham, Oregon. The primary care provider hopes to expand as part of Vancouver Clinic’s continued investment in value-based care.

Humana Well Versed in Value-Based Payments for Medicare Advantage

This is not Humana’s first venture into value-based payment contracts for the Medicare Advantage population. Starting January 1, 2022, the payer began a multi-year, Medicare Advantage value-based care contract in Minnesota. Through a partnership with Allina Health, Humana will extend value-based care in Minnesota to members of Medicare Advantage.

“This value-based agreement for Humana Medicare Advantage members is an important part of helping our members achieve their best health,” Chuck Dow, vice president and Medicare regional president for Minnesota at Humana. “We’re excited to share with Allina Health a strong commitment to providing quality care while improving patient health outcomes in Minnesota.”

It is clear that Humana is committed to expanding through strategic local partnerships to bring value-based care to the Medicare Advantage community. As of September 30, 2021, Humana had over 3.1 million Medicare Advantage and commercial members in more than 1,000 value-based relationships. This spans 44 states and Puerto Rico.
