Blue Cross NC Launches Patient-Centered Kidney Care Value-Based Care Model
Blue Cross Blue Shield of North Carolina announced a new value-based care program designed to improve health outcomes for members with kidney disease.
February 4, 2021 – Blue Cross Blue Shield of North Carolina announced a new value-based care program designed to improve health outcomes for members with kidney disease. They aim to do this through patient-centered care.
Launched January 1, this is the newest feature in the Blue Premier value-based care model from BCBS. Von Nguyen, MD, the Blue Cross NC chief medical offer stated, “It’s important that we help our members with chronic conditions receive coordinated care, delay or prevent the need for dialysis, and improve their quality of life. Advanced kidney disease has long been challenging to treat. Working with FMCNA and Strive Health, Blue Cross NC is bringing an advanced level of care to help our members who need it most.”
The benefits of Blue Premier Advanced Kidney Care include:
- Care management and disease education
- Improved care coordination among primary care and other specialties
- Value-based payments based on provider accountability for the cost of care and quality performance measures
