April 6, 2021 – The Center for Medicare and Medicaid Innovation (Innovation Center) announced the Global and Professional Direct Contracting (GPDC) Model Participants for Performance Year 2021.

The Innovation Center chose 53 Direct Contracting Entities (DCEs) to participate in the first Performance Year (PY2021) of the model. Performance Year 1 of the GPDC Model runs from April 1, 2021 through December 31, 2021.

The Innovation Center also announced it is no longer accepting applications from new organizations interested in participating in the GPDC Model for the period beginning January 1, 2022. This includes Medicaid Managed Care Organization-based Direct Contracting Entities. Organizations that previously applied but deferred their start date to January 1, 2022 will still be permitted to start as planned, as long as they continue to meet GPDC Model requirements.

Global and Professional Direct Contracting Model Details

The GPDC Model is aimed at reducing expenditures while enhancing quality of care for beneficiaries in Medicare fee-for-service. It designed to test various financial risk-sharing arrangements.

The model leverages lessons learned from other Medicare ACO initiatives like the Next Generation ACO Model and Medicare Advantage. It is part of the CMS Innovation Center strategy to drive health care delivery system reform through primary care.

GPDC Model Goals

There are four main goals of the Global and Professional Direct Contracting Model. The first is to transform risk-sharing arrangements in Medicare fee-for-service. This is accomplished by offering capitated and partially capitated population-based payments that encourage a shift away from traditional FFS.

The next goal is to broaden participation in Innovation Center models. This is followed by empowering beneficiaries to engage in their own healthcare through voluntary alignment. The GPDC Model also strives to reduce provider burden to meet health care needs effectively with a smaller set of core quality measures.

CMS encourages those interested in the Global and Professional Direct Contracting Model to follow it’s designated web page for updates.
